Your First Visit

Welcome to Lake Shore Dermatology!  We aim to provide outstanding care, tailored to your skin care needs, in a warm, professional, and caring setting.

What to bring to your first visit

To enable us to facilitate a smooth registration process, please arrive 15 minutes prior to your scheduled appointment time, prepared with the following documents, for your first visit:
• A valid photo ID
• Your current insurance card(s)
• Medication list, including all prescription and over-the-counter medications, vitamins and supplements.
• A list of any and all over-the-counter and prescription medications used to treat your skin concern
• Pharmacy name and address
• Laboratory and/or biopsy results pertinent to your skin concern
• Payment in the form of cash, check or credit card (Visa, Mastercard, Discover) for copays, which will be collected at the time of check in
•    Please refrain from bringing food or beverages with you and please silence electronic devices.

What to do prior to your first visit

Please contact your insurance provider to confirm your insurance benefits and to verify that Dr. Kavitha Gandhi/Lake Shore Dermatology is a provider within your insurance plan’s physician network.

Please use our Patient Portal to digitally complete required medical forms prior to your appointment.  If you did not provide us with your email address at the time you made your initial appointment, please contact our office to provide us with a valid email address so we may send you a unique login and password to the patient portal. If you are not able to access the Patient Portal prior to your first visit with Lake Shore Dermatology, please download, and complete the  New Patient Information/ Medical History Questionnaire form and bring with you to your first visit.

Please download, complete, and bring the following forms with you to your first visit:
 Consent for Release and Use of Confidential Information and Receipt of Privacy Practices 
Financial Policy
• Notice of Privacy Practices (can be downloaded for your records)

If you are unable to complete the above, please arrive at least 15 minutes prior to your scheduled appointment time to do so in the office.  Please note that you will be asked to verify digitally completed information and to complete any remaining paperwork at the time of registration.

All patients under the age of 18 are required to attend the first appointment with a parent or legal guardian.  

Your Follow Up Visit

Welcome back to Lake Shore Dermatology!  We look forward to seeing you again soon in our office and thank you for choosing Lake Shore Dermatology.

What to bring to your follow up visit

To enable us to facilitate a smooth check-in process, please be prepared with the following documents, for your follow up visit:
• A valid photo ID (if we don’t have a digital image of you on file in our electronic medical record)
• Your current, valid insurance card(s)
• Medication list, including all prescription and over-the-counter medications, vitamins and supplements.
• A list of any and all over-the-counter and prescription medications used to treat your skin concern
• Pharmacy name and address (if this has changed from your initial visit)
• Laboratory and/or biopsy results pertinent to your skin concern
• Payment in the form of cash, check or credit card for copays and balances from prior visits, which will be collected at the time of check in
•    Please refrain from bringing food or beverages with you and please silence electronic devices.

What to do prior to your follow up visit

Please use our Patient Portal digitally complete required medical forms prior to your appointment. If you did not provide us with your email address at the time you made your initial appointment, please contact our office to provide us with a valid email address so we may send you a unique login and password to the patient portal.
If you are not able to access the Patient Portal prior to your follow up visit with Lake Shore Dermatology, please download, and complete the following medical questionnaire and bring with you for the follow up visit.
Review of Systems

Please note that you will be asked to update any demographic information, update insurance information, verify digitally completed information, and to complete any remaining paperwork at the time of registration.


All patients under the age of 18 are required to attend the first appointment with a parent or legal guardian.  If for some reason a parent or legal guardian is unable to accompany a patient under the age of 18 for a follow up appointment, please complete the following form and bring with you to your appointment.

Insurance and Billing

As a courtesy to you, we participate in a number of insurance plans.  Please contact your insurance company prior to your appointment to verify that Dr. Kavitha Gandhi/Lake Shore Dermatology is in your network of participating providers.

A current, valid insurance card must be presented at check-in prior to your initial appointment and each time you visit our office. We will verify your benefits prior to your appointment and any copays, coinsurances and unmet deductibles will be collected at the time of check-in.  If we are a participating provider within your insurance plan, we will bill your insurance plan for non-cosmetic dermatology services only.

You will be asked to pay for your charges at the time of service if we cannot verify that your insurance plan is active, or if we are not in network with your insurance plan. An itemized receipt will be provided for you to submit to your insurance company, but we cannot guarantee reimbursement from your plan.

It is your responsibility to understand your individual coverage or benefits including deductibles, coinsurance, and copays. For clarification on your policy’s benefits, please contact your insurance carrier.

If your insurance carrier requires a referral form from your primary care physician and validated by your insurance carrier for a visit to our office, this must be presented at the time of service. Visits that are unauthorized by your PCP and insurance company for specialty care may be denied by your managed care plan and you will be held financially responsible for the services.  It is your responsibility to keep track of referral expiration dates and the number of visits given by your physician.

Cosmetic procedures and products will not be billed to insurance plans and fees for these are due at the time of service or sale.
